Our work brings attention to the support gaps that can accompany leaving care. We invite colleges, agencies, and community organizations to examine continuity: who remains available, what resources are accessible, and how a young person knows where to turn.
The timing and conditions of leaving foster care vary. Our focus is the support young people need throughout the transition.
